Admission

General Documents Required

All candidates applying for admission at Madina Institute of Science & Technology (MISAT) Bannu must submit the following general documents along with the admission form. Incomplete documents shall not be accepted and the application may be rejected without any intimation.

  • Duly filled and signed admission application form.
  • Computerized National Identity Card (CNIC) or Form-B of the candidate (attested copy, both sides).
  • Recent passport size photographs (04 copies with blue/white background).
  • Domicile certificate of the candidate (attested copy).
  • Character certificate from the last institution attended.
  • Admission processing fee challan / bank deposit slip.

Documents For Degree Programs

In addition to general documents, candidates applying for BS degree programs (Surgical, Anesthesia, Pathology, Dental) affiliated with Khyber Medical University (KMU) Peshawar must submit the following:

  1. Matriculation (SSC) certificate and detailed marks certificate (DMC) — attested copies.
  2. Intermediate (F.Sc Pre-Medical) or equivalent certificate and DMC with minimum 50% marks — attested copies.
  3. KMU-CAT Test result card / roll number slip (duly appeared in entrance test).
  4. Equivalence certificate from IBCC (for candidates with foreign or non-standard qualifications).
  5. Migration certificate (if applicable) from the previous board or university.

Documents For Diploma Programs

Candidates applying for two-year diploma programs affiliated with Faculty of Paramedical and Allied Health Sciences (FPMA) Peshawar must submit the following documents in addition to the general requirements:

  • Matriculation (Science) certificate and detailed marks certificate (DMC) — attested copies.
  • Intermediate certificate and DMC (if passed) — attested copies.
  • All previous FPMA DMCs (for candidates applying for renewal or additional diploma).
  • Prescribed fee bank deposit slip as per FPMA rules.
  • Completely filled proforma in the prescribed manner issued by MISAT admission office.

Attestation and Submission

All photocopies of documents must be attested by a Grade 17 or above government officer. Original documents shall be presented at the time of interview and enrollment for verification. MISAT shall not be responsible for documents lost in transit.

  • Submit documents at MISAT admission office, Bagh-E-Yarran, Kohat Road, Bannu during working hours (Mon–Fri, 8:00 am – 4:00 pm).
  • Bring original documents when called for verification after merit list display.
  • Keep one complete set of photocopies for your personal record.
